Using the New Screening System

All staff, visitors, and vendors, or anyone entering the facility are required by the CDC and the State of Minnesota to screen for COVID-19 before each shift and before entering the rest of the facility. This includes answering screening questions and getting your temperature taken before entering the rest of facility to work. You may answer the screening questions on your phone before entering the building at this link, where it will give you a QR code to scan upon entry. This will help avoid waiting in long lines to be screened.

You will be required to enter your phone number and the last 4 digits of your employee number. The last four digits of your employee number are your “PIN”. If you do not have access to a smart phone to answer your screening questions, you may use the larger desk kiosk just inside the employee entryway to answer the screening questions. The smaller kiosk is used to verify that your temperature is in an acceptable range to work at the facility. Any temperature over 100.0 F is considered a fever and you should immediately leave the building, then notify staffing and infection control. You will be asked further questions and may be advised to get tested for COVID-19, and given a return-to-work date once you are fully assessed.

If you have questions on how to use the screening equipment, please call 157 from the screening area, email rderinger@bayviewnursingmn.com, or see Staff Development and Infection Control located on 1E. If you need your employee number, please contact Human Resources by emailing tday@bayviewnursingmn.com.
